Service Development Projects

  • Miniaturized cardiopulmonary bypass (MECC): I was involved with the introduction of miniaturized cardiopulmonary bypass (MECC) for patients undergoing coronary artery bypass grafting (CABG) at Papworth Hospital. I audited the clinical outcome and performed a cost analysis that showed that MECC was associated with less transfusion requirements, higher haematocrit and was cost effective as well. Following the results of my initial audit we performed more CABGs with MECC. I have reaudited the results comparing it with patients undergoing CABG with conventional cardiopulmonary bypass. The results of our reaudit confirm out initial finding that MECC is associated with less haemodilution with reduced need for postoperative blood transfusion.
  • Pulsatile cardiopulmonary bypass: I was involved with the initial study to investigate the benefits of pulsatile cardiopulmonary bypass following its introduction at Papworth Hospital. The results of our study have shown that the patients who had pulsatile cardiopulmonary bypass during open-heart surgery had less incidence of renal failure needing renal replacement therapy. Following the findings of our study pulsatile cardiopulmonary bypass is routinely used for patients with renal dysfunction.
  • The Endocarditis Team: The management of patients with endocarditis is not only challenging but also the management protocol is variable, particularly with regards to the timing of surgery. According to the European Society of Cardiology Guidelines and the AATS expert consensus guidelines there should be a dedicated “Endocarditis Team” for the management of such patients. At Papworth Hospital there is currently no dedicated team for management for such patients. I have recently submitted a proposal with the concept of formation of a multidisciplinary endocarditis team that will consist of cardiac surgeons, cardiologists, microbiologists, neurologists, anaesthetists, intensivists and specialist nurses. This team will have regular multidisciplinary meeting in order to discuss such challenging cases. Hopefully the regular discussion of such cases will improve the management of these patients.
  •  Introduction of phase of Care Mortality Analysis: I have played a pivotal role in the introduction of detailed and structured analysis (similar to phase of care mortality analysis POCMA) of postoperative deaths at our hospital. This has been formally introduced at our clinical governance meeting. I have liaised with the clinical governance lead of the hospital regarding this and have formally introduced it in.
